Overview

Housing Counseling Services is seeking a social worker to coordinate the delivery of services to individuals participating in our agency’s transitional housing program. The social worker will provide case management services to assist program participants identify and implement strategies that support long term housing stability as well as provide assistance in the development and implementation of a plan to respond to presenting housing, financial, health, and other related barriers. As appropriate, the social worker will also assist clients with accessing available community resources to address identified barriers. We are seeking candidates who are smart, high energy, able to problem solve, and committed to helping people to join our housing services delivery team.

PRIMARY DUTIES:

  • Coordinate the delivery of supportive services and case management towards assisting transitional housing program participants move to stable long-term or permanent housing opportunities
  • Assist program participants explore all viable stable housing options including renting, shared housing, and permanent supportive housing options
  • Conduct regular assessment of participants’ needs and barriers. Connect program participants with needed support service including financial assistance, food, utilities, clothing, employment training and opportunities, education services, medical care, etc.
  • Advocate on the behalf of program participants with landlords, social service agencies, etc.
  • Complete housing plans with program participants to detail tasks to be accomplished that support transitioning to permanent housing and to prevent future housing crises
  • Develop household budget and personal money management plans with program participants that support future housing independence
  • Conduct regular home visits and follow-up with participants to evaluate implementation of housing plans and to provide appropriate technical assistance.
  • Develop relationships with housing and community service providers to further support efforts to address program participants’ presenting barriers
  • Maintain accurate written and computer-based files
  • Assist other staff in meeting the needs of HCS clients.
  • Perform other HCS tasks as requested.

BASIC QUALIFICATIONS:

  • Master of Social Work degree – LGSW or higher social work licensure preferred
  • Experience and demonstrated success delivering services to at risk populations
  • Ability to work independently and as a team player, exercising mature judgment
  • Highly motivated self-starter with an ability to complete multiple tasks in timely manner
  • Must have excellent problem-solving skills and be able to develop innovative solutions
  • Must demonstrate the ability to communicate with a variety of professionals including real estate professionals, attorneys, medical providers, etc.
  • Must maintain and manage confidential information
  • Strong oral and written communication skills as well as excellent computer skills
  • Ability to translate technical information into layman’s terms
  • Sensitivity to the needs within the community and to individual clients
